Reporting an incident
Beginning January 25, 2021, the process to submit referrals to the Office of Student Conduct has been updated. Please contact firstname.lastname@example.org if you have any questions.
BEFORE YOU COMPLETE ONE OF THE FORMS BELOW!
If you have knowledge of an incident that is currently taking place, please contact someone immediately. Incidents occurring within Mason residence halls can be reported to Eisenhower Desk at 703-993-2750 or Piedmont Desk at 703-993-9880. All other incidents involving Mason students can be reported to George Mason Police at 703-993-2810. Anonymous tips can also be submitted online at https://police.gmu.edu/contact-us/submittip/.

This form is appropriate for current students, staff and faculty with access to their G#. Please provide as much detail regarding the parties involved and the incident as possible.
All others may submit an incident report here.
